Working together toward a common goal, our staff served nearly 2,700 clients last year. West Central is the nonprofit community mental health care provider for Sullivan County and the Upper Valley, ensuring access to advanced counseling, treatment, and support for clients, regardless of their ability to pay.

As a case manager at West Central Behavioral Health you can be part of this team effort! 

Join our Adult Assertive Community Treatment (ACT) team. This dynamic ACT treatment model provides intensive clinical treatment services to individuals with severe mental illness in a primarily community-based setting. In this role, you will work as part of a supportive, multi-disciplinary team that utilizes a range of evidence-based treatment modalities to help clients with severe and persistent mental illness, work towards their treatment goals.

If interested in a flexible schedule there may be opportunities for weekend or/and call work! 

Responsibilities:

  • Monitor, evaluate and record client progress with respect to treatment goals.
  • Develop objectives and strategies.
  • Communicate with supervisors, peers, or subordinates.
  • Provide case management, IROS and IMR services to assigned case load, both in the office and community sites in keeping with agency and program missions so that their community tenure and quality of life is improved.
  • Assist clients in managing daily problems, stresses, developing a new life structure via work or other meaningful activities, addressing and developing life and illness management skills, and providing family education and support.
  • Coordination of services with other agencies, application and management of benefits, developing and implementing a treatment plan reflecting an outcome orientated and function based philosophy.
  • Work as a team member with other case managers to ensure that comprehensive and quality care is provided to clients at all times.
  • Maintain all documentation that is required in a complete and timely manner.
  • Provide all clinical services in ways that are outcome orientated, recovery based philosophy that is both effective and efficient.
  • Provide crisis intervention as required including, but not limited to, involuntary hospitalization and risk assessment.

Requirements:

  • Bachelor's degree is preferred in human services field
  • 3-5 years experience in mental health, preferably crisis
  • Case management experience is helpful
  • Excellent driving record
  • Reliable and insured vehicle
  • Valid Driver's license
