Team Leader-Adult Case Managers

West Central Behavioral Health is seeking applicants for the Team Leader position for our Adult Case Management Team at our Claremont, NH location.  The individual will be responsible for the management of a team of dedicated individuals.  This includes hire, onboard, supervise, coach, train, and manage performance.

The Team Leader is a member of the management team that involves collaboration with other team leaders around delivery of shared services, communication of new initiatives, and collaboration with outside agencies around shared clinical endeavors.

The Team Leader participates in development of program activities, problem-solve issues and system barriers affecting the delivery of care, and utilization review activities.  The Team Leader manages the overall flow of client care and assigns cases to team members.  Responsible for ensuring staff meet all clinical documentation and productivity goals, accurate and timely billing.

The individual provides services and support to staff members as needed.  The Team Leader will provide some direct services to clients and their families.

General Requirements:

  • Bachelor's degree in social work, rehabilitation, psychology, education or human services field
  • 3-5 years of case management experience in a team setting
  • 2 years of supervisory experience preferred
  • Excellent organizational, communication and customer service skills
  • Possess working knowledge of computers and Microsoft Office
  • Preferred knowledge of and experience with electronic medical records
  • Valid driverís license, plus reliable and insured vehicle
