Case Coordinator -Adults

Case Coordinator-Adults

Claremont, New Hampshire

West Central Behavioral Health (WCBH), Claremont, is seeking a full-time Case Coordinator to join our talented multi-disciplinary team in our Community Support Program working with adults living with severe and persistent mental illness.

As a WCBH Case Coordinator, you'll work closely with our Clinical Team to provide care coordination and case management services for a large caseload of clients whose behavioral health needs consist primarily of in office skills-based therapy.  The primary role of the Case Coordinator will be to assess needs, coordinate care and make referrals to community resources for his/her clients.  The Case Coordinator will also support clinical processes associated with psychotherapy when applicable.  Applicants need to be willing to learn and apply principles associated with Dialectical Behavioral Therapy.
 

Essential Duties and Responsibilities:

  • Help clients and families understand treatment options and make informed decisions
  • Work with treatment team to facilitate case coordination and information sharing
  • Participate in the development and implementation of treatment plans and other clinical documentation
  •   Promote effective utilization of clinical resources, community resources and other social supports
  •  Care coordination/communication both internally and externally

 

Requirements:

  • Bachelorís degree in Psychology, Social Work, Education, Rehabilitation, Human Services or related field is required with 2 years of experience preferred
  • Valid driverís license, personal vehicle, and auto insurance
  • Excellent communication, problem-solving, time management, and organizational skills is required.
